1. Acceptance of These Terms
These Terms of Service ("Terms") are a binding agreement between you and Who We Play For, Inc., a
Florida nonprofit corporation ("Who We Play For," “WWPF,” "we," "us," or "our"). They govern your
access to and use of the Who We Play For results portal, including its login pages, result views,
downloadable reports and letters, and related content and functionality (together, the "Portal"). By
registering for a screening, logging in to the Portal, or otherwise accessing or using the Portal, you agree
to these Terms and acknowledge our Privacy Policy. If you do not agree to these Terms, do not use the
Portal.
2. What the Portal Is
The Portal is a secure website that allows a parent or legal guardian of a screened participant, or a
screened participant who is at least 18 years old, to view the results of an electrocardiogram (ECG) heart
screening performed through a Who We Play For screening program and to download related reports
and letters. The Portal is provided as part of the screening program, and there is no separate charge to
use it.
3. No Medical Advice; Screening Limitations; Emergencies
The Portal is a delivery mechanism for screening results. It does not provide medical advice, diagnosis, or
treatment, and your use of the Portal does not create a physician and patient relationship between you
and Who We Play For. Results available through the Portal are provided for informational purposes and
are not a substitute for evaluation, diagnosis, or treatment by a qualified healthcare provider. Always
seek the advice of a physician or other qualified healthcare provider with any questions about a
screening result or a medical condition, and never disregard or delay seeking professional medical advice
because of information available through the Portal.
An ECG screening is a limited test. It cannot detect every heart condition, and a normal screening result
does not guarantee the absence of a heart condition or rule out future cardiac events. If a result
indicates that follow-up is recommended, you should promptly schedule an evaluation with a qualified
healthcare provider.
If you believe the participant is experiencing a medical emergency, call 911 or go to the nearest
emergency room immediately. Do not use the Portal to seek help in an emergency.

4. Eligibility and Accounts
The Portal is intended for use by (a) parents and legal guardians of screened participants and (b)
screened participants who are at least 18 years old. You may access results for a participant only if you
are that participant, you are that participant's parent or legal guardian, or you are otherwise legally
authorized to receive the participant's health information. You represent that the information you
provided at registration is accurate and complete, and you agree to keep it current. We may suspend or
end access if we believe registration information is inaccurate or that access is unauthorized.
An authorized representative, such as a school or athletic program representative, may access results for
a participant based on consent provided by the participant or, for a participant under 18, the
participant's parent or legal guardian. Authorized representatives may use results only for the purpose
for which consent was given and must keep them confidential.
5. Login Security and One-Time Passcodes
Access to the Portal is protected by a one-time passcode ("OTP") delivered to the email address
associated with your registration and, where text message delivery is available and you have opted in, to
your mobile phone number. You are responsible for keeping OTP codes confidential and for all activity
that occurs through your access. Do not share OTP codes with anyone. Who We Play For will never
contact you to ask for an OTP code. If you believe your access has been used without authorization,

6. Consent to Electronic Communications
By registering for the screening program and using the Portal, you consent to receive communications
from us electronically, including by email and, if you provide a mobile phone number and opt in, by text
message. You agree that agreements, notices, disclosures, and other communications we provide
electronically satisfy any legal requirement that the communication be in writing. You may withdraw
consent to text messages as described in Section 7. Withdrawing consent to email may prevent us from
delivering results notifications and login codes, and you should contact us at info@whoweplayfor.org to
arrange another way to receive results.
7. Text Message (SMS) Program Terms
This section governs text messages sent in connection with the screening program and the Portal (the
"SMS Program"). By providing your mobile phone number and opting in, you consent to receive the
following categories of messages at that number: (a) notifications that screening results are available on
the Portal; (b) one-time passcodes used to log in to the Portal, where text message delivery is available;
and (c) service messages related to your screening, such as notice that a screening needs to be repeated.
Message frequency varies based on your screening activity. Message and data rates may apply. Consent
to receive text messages is not a condition of participating in a screening or of receiving screening
results. If you do not opt in, notifications and login codes will be delivered by email.

Reply STOP at any time to stop receiving text messages from the SMS Program. After you reply STOP, we
may send one final message confirming that you have been unsubscribed. Reply HELP for help, or
contact us at info@whoweplayfor.org or 888.906.9973. Opting out of text messages does not affect
delivery of notifications and login codes by email. Wireless carriers are not liable for delayed or
undelivered messages.
8. Privacy
Our collection, use, and disclosure of personal information, including health information, in connection
with the screening program and the Portal are described in our Privacy Policy. Please review that

9. Permitted Use
You agree to use the Portal only for its intended purpose and in compliance with applicable law. You
agree that you will not: (a) access or attempt to access results for any participant you are not authorized
to access; (b) share your access or OTP codes with others; (c) interfere with or disrupt the Portal or its
security features; (d) probe, scan, or test the vulnerability of the Portal or attempt to bypass any
authentication measure; (e) use automated means to access the Portal or collect information from it; (f)
upload or transmit malicious code; or (g) copy, modify, distribute, sell, or lease any part of the Portal.
10. Results Content and Availability
We work to make results available promptly and accurately, but we do not guarantee that the Portal will
be available at all times or that its content will be free of errors. Results may be delayed, corrected, or
updated. If you believe a result shown in the Portal is incorrect or does not match information you
received elsewhere, contact us at info@whoweplayfor.org. Downloadable files, including ECG reports
and letters, are provided through secure links that expire after a limited time and may need to be
requested again after a link expires.
11. Intellectual Property
The Portal, including its design, text, graphics, logos, and software, is owned by Who We Play For or its
licensors and is protected by intellectual property laws. Subject to these Terms, we grant you a limited,
revocable, non-exclusive, non-transferable license to access and use the Portal for personal,
non-commercial purposes. Results, reports, and letters relating to your participant are provided for your
personal records and may be shared with the participant's healthcare providers, school, or athletic
program as appropriate.
12. Third Party Services and Links
The Portal is operated with the support of third party service providers, including hosting,
communications, and software development vendors. The Portal and related communications may link

to third party websites, such as resources for locating a cardiologist. We are not responsible for third
party websites or services, and a link does not imply endorsement.
13. Disclaimer of Warranties
THE PORTAL IS PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS. TO THE FULLEST EXTENT
PERMITTED BY LAW, WHO WE PLAY FOR DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING
WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND
NON-INFRINGEMENT, AND ANY WARRANTY THAT THE PORTAL WILL BE UNINTERRUPTED, SECURE, OR
ERROR FREE.
14. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, WHO WE PLAY FOR AND ITS DIRECTORS, OFFICERS,
EMPLOYEES, VOLUNTEERS, AND SERVICE PROVIDERS WILL NOT BE LIABLE FOR ANY INDIRECT,
INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF DATA, ARISING
OUT OF OR RELATING TO YOUR USE OF OR INABILITY TO USE THE PORTAL. TO THE FULLEST EXTENT
PERMITTED BY LAW, THE TOTAL LIABILITY OF WHO WE PLAY FOR FOR ALL CLAIMS RELATING TO THE
PORTAL WILL NOT EXCEED THE TOTAL FEES PAID BY YOU TO WWPF FOR THIS SERVICE IN THE THREE (3)
MONTHS PRECEDING THE CLAIM. SOME JURISDICTIONS DO NOT ALLOW CERTAIN LIMITATIONS, SO
SOME OF THESE LIMITATIONS MAY NOT APPLY TO YOU. NOTHING IN THESE TERMS LIMITS ANY LIABILITY
THAT CANNOT BE LIMITED UNDER APPLICABLE LAW.
15. Indemnification
You agree to indemnify and hold harmless Who We Play For and its directors, officers, employees,
volunteers, and service providers from and against claims, damages, and expenses, including reasonable
attorneys' fees, arising out of your misuse of the Portal, your violation of these Terms, or your violation
of the rights of another person, including unauthorized access to another participant's information.
16. Suspension and Termination
We may suspend or terminate your access to the Portal at any time if we believe you have violated these
Terms, if suspension is needed for security reasons, or if the screening program's use of the Portal ends.
Sections that by their nature should survive, including Sections 3, 11, and 13 to 19, will survive any
termination.
17. Changes to the Portal and These Terms
We may update the Portal and these Terms from time to time. If we make material changes to these
Terms, we will post the updated Terms on the Portal, update the Effective Date above, and, where
appropriate, provide additional notice. Your continued use of the Portal after changes take effect
constitutes acceptance of the updated Terms.

18. Governing Law and Disputes
These Terms are governed by the laws of the State of Florida, without regard to conflict of laws
principles. Any dispute arising out of or relating to these Terms or the Portal must be arbitrated in Florida
first and then may be resolved exclusively in the state or federal courts located in Florida, and each party
consents to personal jurisdiction and venue in those courts.
19. Miscellaneous
If any provision of these Terms is held unenforceable, the remaining provisions remain in full force. Our
failure to enforce a provision is not a waiver of it. You may not assign these Terms. We may assign them
in connection with a reorganization or a transfer of the screening program. These Terms, together with
the Privacy Policy and any posted program terms, are the entire agreement between you and Who We
Play For regarding the Portal.
